http://www.speakeasy.net/ Reasonable price, full speed, *flawless* service, very reasonable TOS which pretty much lets you do anything (run servers, NAT, etc) as long as you don't cause a problem for anyone else, excellent online management system, reduced setup fees if you're willing to plug in the box yourself. I started my DSL order around 9pm on a Saturday night. 5 minutes later I was able to log into their online system, and view the ongoing order log, which includes correspondence beween Speakeasy and Covad. Two weeks later, the self install kit showed up. I plugged it in, and it just worked, zero problems.
